Connect with ARS

Connect with ARS by submitting this Self-Identification Form. This form is for use by any incoming or existing student, including graduate and professional students, continuing education students, and program applicants. Submitting this form is the first step in connecting with ARS for accommodations or for students already connected with ARS to request additional or modified accommodations.

What constitutes a "disability"?

A disability is defined in the Americans with Disabilities Act of 1990 as Amended (2008) and Section 504 of the Rehabilitation Act of 1973 as a physical or mental impairment which substantially limits one or more major life activities. If you currently have a physical or mental condition, a history of such a condition, or a condition that may be considered substantially limiting, you may have a legally defined disability.  

Under Section 504 of the Rehabilitation Act, substantially limiting is defined as being unable to perform a major life activity, or significantly restricted as to the manner, condition, or duration under which a major life activity can be performed, in comparison to the average person. A major life activity is defined as performing manual tasks, caring for oneself, seeing, hearing, walking, breathing, speaking, working, and learning.  

Please note:

  • Students should submit the Self-Identification Form and provide ARS with relevant documentation (as described in the Policy on Student and Applicant Accommodations) at least 14 days before the accommodations are needed. Accommodations cannot be provided retroactively. For example, ARS generally cannot provide accommodations for an exam if a student requests accommodations after the exam date has passed.
  • The submission of the Self-Identification Form with documentation does not guarantee your requested accommodations will be approved. Determinations are made on a case-by-case basis based upon our understanding of how the disability / chronic medical condition impacts you as evidenced in the Self-Identification Form and documentation provided. ARS Staff will also consider whether the accommodation request fundamentally alters the nature of the course or program.
  • ARS Staff may contact you with questions or important instructions about necesessary next steps to process your accommodations request. If you have one, we will use your UNC email address to communicate with you. Please add ars@unc.edu to your 'safe sender' list and be on the lookout for emails from us.

Documentation Submission

  • Please refer to the Documentation Guidelines for guidance concerning documentation.
  • Documentation should be on your provider's letterhead and should include contact information and any professional and/or state registration or licensing credentials.
  • If you are not able to submit documentation at this time, or if you have trouble uploading your documentation using the links below, you may send it to ars@unc.edu after you've submitted this form.
  • If you'd like your licensed healthcare provider to submit documentation on your behalf, you can ask them to either complete the Provider Documentation Form, email the documentation to ars@unc.edu, or fax it to us at 919-843-5609. Please make sure your full name or PID is included in the subject line or cover sheet.

Impact Statement

Please use the box below to tell us how your diagnosed disability / medical condition impacts you. Be as detailed and specific as possible in describing the current or recent impacts you've experienced.

Consider these questions to guide you:

  • How does your disability affect you academically? How do you think it will affect you academically at UNC?
    • Think about skills such as reading, writing, math, paying attention, studying, completing various types of assignments, taking tests, group work, presentations, labwork, fieldwork, etc.
  • How does your disability affect your daily life in general? How do you think it will affect your daily life at UNC?
    • Think about your life at home, getting around campus, time management, etc.

Please note:

  • For housing accommodations you must also adhere to Carolina Housing's processes, including applying for housing and adhering to their deadlines.
  • Requests for housing accommodations received after the applicable deadline(s) will be considered but our capacity to grant reasonable accommodations will be severely impacted by availability.
  • Housing accommodations are assigned to a qualified individual only and do not extend to roommates.


Clinical / Placement Accommodations

Emotional Support Animal

Provider documentation accompanying an ESA request should include statements specific to:

  1. ​the existing and continuing diagnostic and therapeutic relationship you have with your provider;
  2. your provider’s specific diagnosis of the condition and its severity
  3. the methodology used to arrive at this diagnosis;
  4. ​the functional limitations resulting from this diagnosis and a clear articulation of how the animal will mitigate these functional limitations;
  5. the observations, evaluations or means by which the provider has determined that an ESA would be a reasonable accommodation;
  6. the provider’s confirmation that the ESA has been prescribed for treatment purposes and as part of a care or treatment plan and is necessary to help alleviate symptoms associated with your condition and how it will be critical in allowing you to use and enjoy university housing services.

Confirm and Submit

By providing my UNC PID and ONYEN below I affirm that I am electronically signing this document and acknowledge that:

  1. Only I, the student or applicant, have filled out this form;
  2. The submitted request will not be processed until pertinent documentation of disability has also been provided;
  3. Details in this form and any attached documentation may not be adequate for determination of eligibility for the requested accommodations;
  4. The accommodation determination process may take up to 14 days after all necessary information, including documentation, is received;
  5. The Accommodations and Modifications Committee will consult as part of the review process and can only consider the accommodations as requested on this form;
  6. Accommodations, if rendered, may not be the same as those I received in high school or at another institution, and will not apply retroactively;
  7. There may be deadlines after which accommodation requests related to Final Examinations cannot be processed.
